What is Plecanatide?
Plecanatide is a new drug that affects guanylate cyclase, which can stimulate the intestinal secretion of intestinal juice and accelerate bowel movements. An oral polypeptide composed of 16 amino acids, designed to mimic the human gastrointestinal peptide-uroguanylin. Pukanatide was originally developed by Callisto Pharmaceuticals in the United States and transferred to Synergy Pharmaceuticals for development.
It was approved by the U.S. Food and Drug Administration (FDA) on January 19, 2017, for the treatment of adult CIC. Pukanatide is an analog of uroguanylin (uroguanylin). It contains a 16 amino acid cyclic polypeptide. It acts as a natriuretic guanylate cyclase receptor agonist and can regulate the gastrointestinal tract. Acid-base ions induce fluid transport into the gastrointestinal tract and increase the peristalsis of the gastrointestinal tract.
Plecanatide’s mechanism of action
Plecanatide and its active metabolites bind to the GC-C enzyme and act locally on the lumen surface of intestinal epithelial cells. The activation of the GC-C enzyme increases the concentration of cyclic guanosine phosphate (cGMP) inside and outside the cell, while the concentration of cGMP inside the cell increases, mainly through the activation of the ion channel of the cystic fibrosis transmembrane regulator (CFTR). Stimulate the secretion of chloride and bicarbonate into the intestinal lumen, increasing the small intestinal fluid.
Indications of Plecanatide (Prikanatide)
It is suitable for the treatment of adult CIC patients.
What is Plecanatide (Prikanatide) mainly used for?
Prakanatide is a natriuretic guanylate cyclase C (guanylate cyclase-C, GC-C) receptor agonist, which can regulate acid-base ions in the gastrointestinal tract, induce fluid transport into the gastrointestinal tract, and increase The peristalsis of the gastrointestinal tract is suitable for the treatment of adult CIC.
Adverse reactions related to Plecanatide (Prikanatide)
The adverse reaction of Prikanatide is lower than that of linaclotide. The removal rate caused by severe diarrhea in clinical trials is much lower than that of linaclotide, and in a clinical trial for the treatment of CIC, Prikanatide diarrhea The rate is <6%, while linaclotide is 16%. The domestic prevalence of digestive diseases is much higher than that in Europe and the United States, and the domestic prevalence of the indications targeted by Prikanatide is higher than that of European and American countries
So Prikanatide has greater market potential in the future. This medicine is forbidden for children under 6 years of age because of the potential risk of severe dehydration and may cause diarrhea. Patients between the ages of 6 and 18 should also avoid the use of Prikanatide because the safety and effectiveness of Prikanatide in this age group have not been verified by clinical trials. Prikanatide should not be used for patients suffering from or suspected of mechanical gastrointestinal obstruction. Prikanatide can be taken with or without food.
